Direct Referral: To refer a patient, simply fax a prescription or the “Prosthetic Referral Form” found on the FORMS page, and a completed patient demographic form to Mosaic Prosthetic Solutions. Alternatively, you may contact our office directly, and our team will gladly assist you. Once we receive the required information, we will promptly reach out to the patient to schedule a consultation and evaluation.
We welcome and encourage patients to contact us directly to schedule an appointment. Please note that a valid prescription is required before any services, adjustments, or evaluations can be provided. In the absence of a prescription, the appointment will be limited to a consultation.
